Kernel crash via stale SCSI command flush

NULL pointer dereference in the qla2xxx SCSI driver in Linux 4.11 and later allows attackers to crash the kernel after a cable pull. During session teardown, a failed SRB allocation leaves commands unflushed while the SCSI layer modifies scsi_cmnd; a later flush then DMA-unmaps a null SGL. Reachability requires memory stress, an active qla2xxx Fibre Channel session, and a subsequent cable pull, and the observed consequence is a system crash.
