System crash via concurrent SGE release

Race condition in the Linux bnx2x driver allows local users to crash affected systems during PCI EEH error recovery. bnx2x_nic_unload and bnx2x_io_slot_reset can concurrently release the same receive SGE, leaving bnx2x_free_rx_sge to read a page pointer cleared after dma_unmap_page. Exploitation requires local access, a bnx2x network device, and overlapping transmit-timeout and EEH reset processing; the bounded consequence is a kernel crash.
