LPAR crash via uninitialized IOMMU table

NULL pointer dereference in affected Linux kernel versions allows local users to crash a PowerPC pSeries LPAR during kdump over SR-IOV. iommu_table_setparms_lpar() considers only the absent ibm,dma-window property, leaving a dynamically mapped DDW table uninitialized before iommu_area_alloc() reads through it. Reachability requires administrative control of kdump on a pSeries LPAR with an SR-IOV device and a dynamic DDW; the panic interrupts crash-dump collection.
