Host hang via inverted pKVM lock ordering

Deadlock in KVM arm64 allows local users to hang affected Linux hosts through protected-VM creation during a vCPU ioctl. pkvm_create_hyp_vm() takes kvm->lock while kvm_vcpu_ioctl() already holds vcpu->mutex, reversing KVM's required order and permitting a circular wait. Reachability requires local low-privileged access to KVM on an arm64 host configured for protected virtualization; the bounded consequence is a host hang.
