Client data exposure via discarded outer-scope headers

Incorrect header inheritance in h2o through commit 5f5cd8e allows remote attackers to expose clients to unsafe responses via nested directives. The headers configurator's on_config_exit path registers an inner-scope command list without retaining outer-scope definitions, so nested actions replace inherited header actions. Exploitation requires the affected nested configuration and client interaction; omitted security headers can remove browser mitigations, with no reported integrity or availability impact.
