Map URL denial via sequential identifier collision

Improper input validation in the Zabbix API and frontend allows remote authenticated users to block new map-element URLs. When a URL is added, the code derives its sequential sysmapelementurlid from the last database value without rejecting a user-supplied next identifier, creating a collision. Administrative access sufficient to alter map URLs is required, and the collision prevents other users from adding URLs.
