Account takeover via password-reset MFA bypass

Authentication bypass in RubyGems.org before commit 0b3272a allows remote attackers to take over MFA-enabled accounts via password reset. PasswordsController accepted a direct password-update submission with only a valid confirmation token instead of enforcing the MFA verification flow. Exploitation requires control of the account's email or its forgotten-password token and changes the password, granting control of the account.
