Account persistence via unrevoked access tokens

Improper authentication in Memos 0.18.1 and earlier allows remote authenticated users to retain account access after a password change. The password-change flow leaves existing Access Tokens in the valid-token list instead of revoking tokens issued under the old password. Possession of a previously issued token is required; the stale token preserves the compromised account's existing privileges despite the credential reset.
