Stored XSS via unsanitized CSV username

Stored XSS in IP Based Login before 2.4.1 allows remote authenticated users to inject script through a crafted CSV import. The import handler accepts an HTML-bearing username from the CSV without sanitization, stores it as an IP-range entry, and renders it on the plugin admin page. An administrator must import the file, and the script runs when the page reloads even if the unfiltered_html capability is disabled.
