Reviewer assignment changes via missing CSRF checks

CSRF in PKP Web Application Library before 3.3.0-16 allows remote attackers to alter reviewer assignments via forged requests. ReviewerNotifyActionForm fails to register FormValidatorCSRF for reviewer cancellation and reinstatement submissions, so requests lacking a valid token are accepted. Exploitation requires an authenticated editor's browser session, and success cancels or reinstates an assigned reviewer.
