Stored XSS via unescaped IP request headers

Stored XSS in burningx mpOperationLogs 1.0.1 and earlier allows remote attackers to inject scripts through IP request headers. The plugin stores client-controlled IP header values in its login-IP log and renders them in the administrative IP list without adequate sanitization or output escaping. No authentication is needed to supply a crafted header, but a user must open the affected log page before the script executes in that site's browser context.
