Out-of-bounds write via unbounded host string formatting

An out-of-bounds write in the Linux qla2xxx SCSI driver allows attackers to cause unspecified impact via unspecified vectors. In drivers/scsi/qla2xxx/qla_os.c, sprintf formats a host identifier into the 16-byte vha->host_str array without bounding the output, so indexes 16 through 19 can write past the array. Public upstream records identify neither an attacker-controlled input nor a demonstrated consequence beyond the out-of-bounds write, so reachability depends on the qla2xxx driver path.
