Kernel crash via stale dcssblk xarray entries

Denial of service in the Linux kernel dcssblk driver allows attackers to crash s390 systems via repeated device add and remove cycles. Missing dax_remove_host calls leave stale gendisk pointers in an xarray, and a reused pointer drives dax_add_host into an error path with an unmatched put_device. The flaw requires the s390 dcssblk driver and device lifecycle operations; no remote trigger or privilege boundary is published.
