Protected-VM crash via uncleared ASCE page index

Improper page-index handling in Linux KVM s390 allows local users to crash a protected virtual machine during ASCE replacement. The s390_replace_asce function fails to reset the replacement guest ASCE page index to zero, causing PTE invalidation notifications to carry incorrect addresses. Triggering the crash requires s390 protected virtualization, replacement of a guest ASCE, later unmapping of the prefix, and notifier execution.
