Kernel crash via unallocated replacement buffer dereference

Null-pointer dereference in Linux md/raid10 allows local users with storage-administration privileges to crash the kernel during array recovery. A skipped recovery calls init_resync() but not close_sync(), leaving have_replacement cleared so raid10_sync_request() fails to allocate repl_bio before dereferencing it. Reachability requires a completed RAID10 resync, a skipped recovery, I/O errors, and addition of a replacement device before recovery runs again.
