Debug-kernel crash via duplicate session deletion

A race condition in the qla2xxx SCSI driver in Linux 4.11 through 6.5.3 allows local users to crash a debug kernel. The driver updates an FC port's deleted flag outside the session lock, allowing two CPUs to enqueue the same session deletion and corrupt the deletion list. Reachability requires the qla2xxx driver, a debug kernel, and concurrent Fibre Channel session deletion; the reported consequence is a system crash.
