Host crash via premature remote-port cleanup

Use-after-free in Linux 2.6.34 through 6.4 allows remote attackers on the same Fibre Channel fabric to crash the host during port loss. The qla2xxx terminate_rport_io path returns before outstanding FCP-2 I/O reaches the upper layer, allowing cleanup to free resources still referenced by the driver. Reachability requires a qla2xxx Fibre Channel HBA, an FCP-2 target with outstanding I/O, and loss of its remote port; delayed I/O completion then causes the kernel crash.
