System hang via out-of-order IOCB accounting

NULL pointer dereference in the Linux qla2xxx SCSI driver allows local users to hang affected systems during NVMe/FC teardown. The qla_nvme_ls_req path encounters out-of-order IOCB accounting, which blocks subsequent commands and reaches a null wake queue. Exploitation requires local low-privileged access and a host using the qla2xxx driver for an NVMe over Fibre Channel association; the bounded consequence is a host hang.
