Code execution via submap array off-by-one write

Out-of-bounds write in stb_vorbis 1.22 and earlier allows remote attackers to execute code via a crafted Ogg Vorbis file. The start_decoder loop permits 16 submaps but writes their floor and residue values into two 15-element Mapping arrays. Exploitation requires a consuming application to decode attacker-controlled audio; successful memory corruption executes code in that application's context.
