Code execution via unchecked ICO palette index

Heap-buffer overflow in OpenImageIO 2.4.12.0 and earlier allows attackers to execute code by processing a crafted ICO file. ICOInput::readimg derives palette indices from pixel data and dereferences the palette without checking each index against m_palette_size. A user must cause an affected application to process the file, after which the out-of-bounds read can expose memory, crash the process, or enable code execution.
