Stored XSS via unescaped form_id setting

Stored XSS in CRM Perks Forms 1.1.1 and earlier allows remote authenticated users to inject scripts through form settings. The settings page renders the form_id parameter without sufficient sanitization or output escaping, preserving attacker-supplied event-handler markup. Administrator access plus multisite or disabled unfiltered_html is required, and the stored script runs when a user opens the injected page.
