Authenticated PHP code execution via practice-logo upload

Unrestricted file upload in NOSH 4a5cfdb allows remote authenticated users to execute arbitrary PHP code through the practice-logo feature. The practice-logo upload handler trusts a browser-side extension check, then accepts a renamed .php upload in a context where PHP executes it. A valid account is required; the executed code runs on the NOSH server and can expose or alter hosted health records or interrupt the service.
