Stored XSS via unescaped form-grid shortcode attributes

Stored XSS in the GiveWP plugin 2.25.1 and earlier allows remote authenticated users to inject scripts through a form-grid shortcode. The give_form_grid handler insufficiently sanitizes and escapes user-supplied shortcode attributes before rendering them into a page. Contributor-level access is required, and the stored script executes when another user views the injected page in a browser.
