Authenticated code execution via unsanitized file inclusion

Local file inclusion in OpenEMR before 7.0.0 allows remote authenticated users to execute code through the formname parameter. interface/forms/LBF/new.php appends formname to an LBF path and includes the resulting .plugin.php file without rejecting traversal sequences. A valid session and a previously uploaded .plugin.php payload are required, after which the included PHP runs with the web server's privileges.
