Stored script execution via unescaped display name

Stored XSS in Easy!Appointments before 1.5.0 allows remote authenticated users to execute scripts through a crafted display name. The backend_header.php view inserts user_display_name directly into HTML; the patch wraps that value in the application's escaping helper. Administrative access is required to set the name, and another backend user's browser must render it; executed code acts in that viewer's session.
