Persistent access via unchecked identity-provider status

Insufficient session expiration in all versions of Cloud Foundry Foundation UAA allows remote authenticated users to retain resource access. The refresh-token grant path fails to check whether the external identity provider that issued the user session remains active and continues exchanging its refresh token for new access tokens. Exploitation requires a refresh token issued before an administrator deactivates that provider, preserving access to Cloud Foundry resources until the token expires, by default after 30 days.
