Kernel panic via duplicate queue_rq dispatch

Race condition in the Linux kernel blk-mq subsystem allows local users to trigger a kernel panic through delayed request queuing. A request can time out after blk_mq_start_request() begins queue_rq but before hardware submission, letting the timeout handler requeue it and invoke queue_rq twice. Triggering the race requires a virtual-machine scheduling delay, vCPU preemption latency, or a long vCPU page fault; the resulting impact is a host kernel panic.
