Host crash via abort-timeout double completion

Denial of service in the Linux qla2xxx SCSI driver allows attackers on an attached Fibre Channel SAN to crash the host kernel. The qla24xx_abort_iocb_timeout path completes an aborting request without first verifying that both the original I/O and its abort request remain outstanding, permitting double completion of a freed request. Reachability requires qla2xxx-backed NVMe over Fibre Channel I/O and an attached SAN target that controls completion timing; the demonstrated consequence is a host-kernel crash.
