Domain crash via stale split-VMA tracking

Denial of service in the Linux kernel xen/gntdev driver allows local users to crash a Xen PV domain via split grant unmapping. The driver retains one VMA reference for a mapping that can span multiple split VMAs and removes its MMU notifier before the final VMA closes, causing invalid page unmaps. The path requires local access to xen/gntdev in a PV domain, and the reported general protection fault occurs when Xen is built with CONFIG_DEBUG.
