Kernel crash via stale xHCI bandwidth-list entries

Double free in the Linux xHCI cleanup path allows local users with administrative privileges to crash affected Panther Point hosts. Early returns during controller removal leave freed endpoints linked in the software bandwidth list, and xhci_mem_cleanup later applies list_del to those stale entries. Triggering the fault requires CAP_SYS_ADMIN access to unbind xhci-pci and a host using Intel Panther Point xHCI software bandwidth checking.
