Local kernel crash via stale KFD cleanup pointer

NULL pointer dereference in AMD KFD process cleanup in the Linux kernel allows local users to crash the system during process teardown. On a CWSR allocation failure, kfd_process_device_init_vm frees outstanding buffer objects without clearing the associated qpd cleanup state, so the later worker dereferences an invalid pointer. Access to the AMD KFD device and a partial VM-initialization failure are required; the documented consequence is a kernel oops or panic.
