Service interruption via leaked Xen grant references

Resource leak in Linux kernel xen/gntdev allows local users to interrupt service by repeatedly triggering partial grant mappings. gntdev_map_grant_pages counts only entries whose map_ops and kmap_ops both succeed, so live_grants omits successful kernel mappings and unmapping skips or over-subtracts them. The condition requires a paravirtualized Xen domain and partial grant-map failures; repeated failures retain grant references and exhaust availability.
