System crash via uninitialized IOMMU domain dereference

Use of an uninitialized resource in Linux 4.6 through 6.0.16 and 6.1 through 6.1.2 allows local users to crash the system. During a kexec reboot, mtk_iommu_isr passes dom->domain to report_iommu_fault before the MediaTek IOMMU domain pointer is initialized. The flaw requires local low-privileged access, a MediaTek IOMMU platform, and the interrupt timing window; successful triggering causes an invalid kernel memory read and system crash.
