Kernel crash via missing decoder cleanup

NULL pointer dereference in Linux 6.0 through 6.0.7 allows local users to crash the kernel while creating a CXL region. The cxl_port_attach_region() cleanup path releases a region reference after decoder allocation fails, then a sanity check dereferences the absent decoder. Reaching the fault requires low-privilege local access, exhausted intermediate-port CXL decoders, and creation of another region; the consequence is a kernel crash.
