Host crash via inactive page-cache refresh race

Use-after-free in Linux KVM 5.17 through 6.0.7 and 6.1 release candidates allows local users to crash the host kernel. The gfn-to-pfn cache helpers accept an inactive cache, letting a refresh mark it valid while its kernel address is NULL and absent from notifier tracking. The race requires low-privileged local access to concurrent KVM Xen ioctls; its demonstrated consequence is a NULL-pointer kernel crash.
