Kernel crash via NULL PCS_USB runtime-resume base

NULL pointer dereference in the QMP combo PHY driver in Linux 6.0 through 6.0.8 allows local users to crash the kernel during runtime resume. The runtime-resume path derives the PCS_USB register base from a separate mapping even when platform data supplies no PCS_USB offset, leaving the base NULL. The condition is limited to platforms such as Qualcomm SC7180 that omit that offset, and triggering it interrupts the host through a kernel oops or panic.
