Kernel out-of-bounds access via offline queue index

Improper array-index validation in Linux kernel 4.16 through 5.18.5 allows local users to read or write kernel memory or crash the host. When an NVMe-oF connection targets an offline hardware queue, blk_mq_alloc_request_hctx() accepts nr_cpu_ids from an empty online-CPU mask and passes it to __blk_mq_get_ctx(), indexing a per-CPU array beyond its bound. Reachability requires local access to initiate an NVMe-oF queue connection while every CPU mapped to that queue is offline.
