Kernel panic via duplicate UVC request enqueue

Double free in affected Linux kernel revisions allows local users to trigger a kernel panic through the USB gadget UVC endpoint. In uvcg_video_pump, an endpoint-disable race leaves a queued request handle valid, so the pump places the request on req_free before the completion callback places it there again. Local access capable of configuring and exercising the USB gadget UVC function is required; the demonstrated consequence is a kernel BUG and system crash.
