I/O hang via uncleared NBD requests

Denial of service in the Linux kernel NBD driver allows local users to hang I/O while disconnecting a configured NBD device. After a failed NBD_DISCONNECT, nbd_clear_sock_ioctl() does not clear inflight requests, while nbd_xmit_timeout() continually resets their timers so they never complete. Reaching the flaw requires local access to issue NBD ioctls; the resulting uninterruptible wait blocks affected NBD operations.
