Storage interruption via premature PCI hardware access

Improper PCI error recovery in the Linux kernel qla2xxx driver allows local users to interrupt storage operations on affected hosts. The qla2xxx_pci_mmio_enabled callback reads adapter registers and can initiate a firmware dump before verifying that the PCI disconnect has cleared or the operating system has signaled resume. Reachability requires a QLogic adapter using qla2xxx and a recoverable PCI error; the observed consequence is failed firmware-dump activity and mailbox-command timeouts.
