Physical-memory access via confused DAX PUD walk

Memory corruption in the Linux kernel allows local users to read or write physical memory through an x86 device-DAX PUD mapping. gup_pud_range() treats a devmap-backed huge PUD as a page-table pointer when HUGETLB is disabled, causing a confused page-table walk. Access to a suitably aligned /dev/dax mapping is required; crafted entries permit page access, while invalid entries panic the host.
