Local boot crash via duplicate IOMMU insertion

Denial of service in the Linux kernel IOMMU subsystem allows local users to crash the system during boot on affected Intel hardware. In dmar_insert_one_dev_info(), VMD subdevices incorrectly receive PASID-table allocation; the failure path frees device-domain metadata that is then reused and added to the list twice. The crash requires Intel VMD and VT-d scalable mode on affected Sapphire Rapids hardware, and the reported consequence is a kernel panic rather than data disclosure or modification.
