Vhost thread hang via zero-size IOTLB mapping

Denial of service in vhost IOTLB handling in affected Linux kernels allows local users to hang a vhost thread via a crafted mapping. vhost_iotlb_add_range_ctx() calculates an inclusive range size that wraps to zero for [0, ULONG_MAX], leaving an erroneous entry that makes iotlb_access_ok() loop indefinitely. A local user able to submit a zero-sized IOTLB mapping through a vhost character device can trigger the hang when a packet is processed.
