Local use-after-free via command-flush race

A race condition in the Linux kernel's mlx5 driver allows local users to trigger a command-entry use-after-free during command flushing. The flush path can acquire a reference after another process drops the entry's last reference but before its index is released because the operations lack a shared spin lock. Reachability requires the mlx5 driver and concurrent command release and firmware-fatal flushing; the documented result is a refcount warning and use-after-free.
