Kernel page fault via stale reply-queue pointer

Invalid-pointer dereference in the Linux kernel allows local users to trigger a kernel page fault during an mpt3sas LUN reset. mpt3sas_base_sync_reply_irqs() calls _base_process_reply_queue() after list_for_each_entry() completes, leaving reply_q outside the traversed list. Local access and an mpt3sas-controlled SCSI device entering the reset error path are required; the resulting fault interrupts service.
