NPIV vport outage via uninitialized delayed work

Improper initialization in the Linux qedf SCSI driver allows local users to interrupt NPIV vport availability during Fibre Channel login. qedf_vport_create copies the physical-function context but fails to reinitialize stag_work, leaving its internal list pointers tied to the original object. The flaw requires a QLogic FastLinQ FCoE configuration with NPIV vports and repeated unanswered login attempts; affected vports can remain permanently offline.
