I/O hang via leaked task-management reference

Denial of service in the qedf SCSI driver in Linux 4.11 through 5.17-rc1 allows local users to hang an I/O during concurrent LOGO and TMF handling. During forced completion in qedf_initiate_cleanup(), the task-management path clears command state and signals completion without dropping the io_req reference. The condition requires a qedf Fibre Channel session to receive LOGO while a LUN or target reset is active, leaving storage I/O blocked rather than corrupting data.
