Guest-to-host privilege escalation via shadow-page use-after-free

Use-after-free in Xen shadow paging in 3.2 and later allows guest OS users to crash the host or escalate privileges during migration. The shadow allocator reserves for page-table demand but omits log-dirty metadata demand, so a new shadow page table can be freed while other code still accesses it. Exposure is limited to x86 PV guests, or HVM and PVH guests using shadow paging, while migration or snapshotting enables log-dirty mode.
