Process crash via unreleased file stream allocation

Memory leak in Bento4 1.6.0 mp4fragment allows local users to consume process memory by supplying a crafted MP4 file. AP4_StdcFileByteStream::Create allocates a 48-byte file-stream object during malformed-file processing but does not release it when that path terminates. Exploitation requires a user to invoke mp4fragment on attacker-controlled input and affects availability by consuming memory until the process crashes.
