Disabled-account access via omitted PAM authorization check

Improper authentication in Arvados 2.4.2 and earlier allows remote authenticated users with disabled PAM accounts to obtain access. The PAM login path validates supplied credentials but fails to check whether PAM permits the account to access the controller host before issuing an Arvados API token. The flaw is reachable only when PAM is the configured identity provider; LDAP and OpenID Connect deployments are unaffected, and accepted accounts gain their configured Arvados access.
